1. Download the application form.
2. Fill up the information in own handwriting.
3. Paste photographs taken from the same negative). Do not staple the Photographs.
4. Forward the Application Form (duly filled in) along with necessary documents and a Demand Draft of Rs.3,750/- (Rupees three thousand seven hundred fifty only) drawn on any nationalized Bank, in favour of 'The Registrar, Dr. D. Y. Patil Vidyapeeth' payable at Pune. The last date of form submission is  Without late fee 09/01/2014
5. Send the Application forms mentioning over the envelop "APPLICATION FORM FOR AIPGMET-2014 " addressed to

   Sant Tukaram Nagar, Pimpri,
   Pune - 411 018
   Maharashtra.

On Receipt of Application Form along with a DD of requisite amount, the Information Brochure will be dispatched at your given address. The Information Brochure Serial Number shall be your Application Form Number for further correspondence, if any.

List of Documents (to be attached to the application form)

The candidate should submit photocopies of the following documents along with the application form:

1. Statement of Marks of I, II, III, MBBS examination
2. Attempt Certificate
3. MBBS Degree/Passing Certificate
4. Internship Completion Certificate
5. Registration Certificate (MCI/State MC)
6. Age and Nationality Certificate
7. Bonafide Certificate from Dean
8. Transfer Certificate
9. Migration Certificate
10. Proof of Date of Birth
11. Two recent photographs, taken from same negative, without cap or goggles, taken within last six months. One should be pasted on the application form, duly attested by the Principal/Dean of the College or any Gazetted Officer. The other should be stapled to the application form.
12. Demand Draft of the requisite amount
13. Authorization Form for such candidates who due to some emergency may not be in position to attend the PG counseling scheduled.
